Enter Macro Commands:
#showtooltip /use <<Your poison>> /use [mod:shift] 17; 16
Replace the highlighted words with your own before copying.
What it does
Uses the poison you name, then applies it to your main-hand weapon (slot 16), or to your off-hand weapon (slot 17) while Shift is held.
How to use
Make one macro per poison (for example Instant on the main hand, Deadly on the off hand), type the poison's exact name, and press it out of combat.

Limitations
- If the weapon already has a poison, the game asks you to confirm the replacement.
Compatibility
How to add a macro in the game
- Type /macro in the chat box and press Enter, or open the game menu and choose Macros.
- Choose New. Give the macro a name and pick the question-mark icon: with #showtooltip the button then shows the spell it casts.
- On this page, choose a macro and press Copy Macro.
- Click into the macro window's text box and paste (Ctrl+V, or ⌘V on a Mac).
- Drag the macro's icon from the window onto an action bar.
General Macros are shared by all your characters on the account; Character Specific Macros belong to one character. Class macros belong in the character tab.
